  • Background

Amiodarone and desethylamiodarone are primarily used for the treatment of supraventricular and ventricular arrhythmias. They are particularly suitable for the management of arrhythmias associated with acute myocardial infarction and acute myocardial ischemia, and are also recommended for patients with heart failure.
Amiodarone additionally possesses anti-anginal properties. It selectively dilates the coronary arteries and reduces myocardial oxygen consumption, making it suitable for patients with structural heart diseases such as coronary artery disease and heart failure. It is also indicated for patients with malignant arrhythmias, such as tachyarrhythmias accompanied by hypotension. As a broad-spectrum antiarrhythmic agent, amiodarone is widely used in clinical practice.
However, amiodarone is associated with potential adverse effects, including thyroid enlargement and pulmonary toxicity. Therefore, therapeutic drug monitoring of amiodarone is essential to support individualized dosing under physician supervision.
